Wir empfehlen allen Hydrozephalus-Patienten bzw. den sie begleitenden Betreuungspersonen, einen Shunt-Pass\/Patientenausweis f\u00fcr Notf\u00e4lle mit sich zu f\u00fchren. Ihr Arzt hat Ihnen diesen sicherlich nach der Implantation oder der letzten Ventileinstellung ausgestellt. Vergewissern Sie sich, dass Ihr Patientenausweis auf dem neuesten Stand ist.<\/span>\u00a0<\/span><\/p>\n

Wenn Sie ein einstellbares Codman Hakim Medos- oder Certas Plus Ventil implantiert haben, aber Ihr Shunt-Pass\/Patientenausweis nicht aktuell ist, k\u00f6nnen Sie einen der nachstehenden Blanko-Ausweise herunterladen und von Ihrem Arzt ausf\u00fcllen lassen. Die Patientenausweise (auch Implantationsausweise genannt) sind implantatspezifisch.
